When the bladder is removed a section of the intestinal tract can be used to create a new bladder or it can be used to create a tube leading from the kidney to the abdomen where a small port is made called a urostomy allowing the kidneys to empty out into an external pouch where the urine collects and can be emptied when necessary. In either urinary diversion, the healing time of of the new bladder, called a neobladder, or the urostomy takes weeks to heal. Bladder training for the neobladder includes building up the muscles at the bottom of the neobladder to maintain continence and relaxing the muscles holding in the urine to urinate. For those with the urostomy, training involves emptying, cleaning and replacing the pouch when necessary along with care of the stoma.
Da Vinci prolapse surgery is also performed when a women gets a dropped bladder which is caused by the supporting tissues in the bladder, uterus, or rectum give way. Kegel exercises are performed early in life to prevent these structural weaknesses from occurring however giving birth, obesity, and straining often during bowel movements are common risk factors for bladder prolapse. Da Vinci prolapse surgery creates a support structure with synthetic mesh sling made with synthetic polypropylene or biologically grafted tissue. Another surgical method is tightening the tissues that support the bladder with sutures.
